Description:

Share UCSF data responsiblyWith advances in treatment there is a growing population of childhood cancer survivors. Survivors are at risk for late effects associated with the life-saving treatments of chemotherapy, surgery and radiation. In one study of survivors and their siblings, it was discovered that 62% of survivors suffered from at least one chronic health condition; with 27% suffering from a life-threatening condition. Survivors have been shown to be at increased risk for developing secondary malignancies, infertility, organ dysfunction, endocrine and metabolic disorders, physical performance problems, psychological and neurocognitive difficulties, amongst other life-altering complications. Unfortunately evidence shows that compliance with survivor follow-up care is less than optimal. Less than half of childhood cancer survivors receive long-term follow-up care. These findings highlight the need to educate survivors regarding their specific treatment-related risks and to improve compliance with ongoing follow-up across their lifespan.

We have been developing a software platform with web & mobile-app interfaces to engage, educate and help childhood cancer survivors navigate highly individualized follow up plans. The platform is designed to gather information from electronic medical records regarding individual survivors’ diagnoses, treatment history, and based on such information automatically formulate individualized follow up plans according to the gold standard survivorship guidelines from the Children’s Oncology Group (COG). A web interface has been built to allow providers at cancer survivorship program to review the care plans and manage the patient panel. A mobile application is being constructed to deliver the individualized plan to the survivor’s mobile device, with features such as calendar reminder and push notification to help them manage follow up appointments.
